LIFE Connection — A Program of All-Inclusive Care for the Elderly (PACE) in New Jersey
PACE is a Medicare and Medicaid program providing aging-in-place long-term care services. LIFE (Living Independently For the Elderly) Connection is the name of the PACE organization at AtlantiCare — from providers to treatments to services and more. Because we are a Medicare and Medicaid program, we function effectively as both the provider and insurer — we cover every service we recommend for your care, so we can more creatively and flexibly meet your needs. We focus on providing services to meet all your care needs and exceeding your expectations.

The goal of hospice care is comfort and compassion. That’s why LIFE Connection offers hospice-level care on the same in-home basis as our other services. We feel this is the best way to provide a positive experience for both the participant and their loved ones.
