LIFE Connection is a hybrid care model for people who are eligible for a nursing facility but wish to remain in their own home. It helps people maintain more independence and allows loved ones to stay involved in their care — with additional caregiver support.

The team working with you to develop your plan consists of:

  • Primary care provider
  • Center nurse
  • Social worker
  • Physical therapist
  • Occupational therapist
  • Recreational therapist
  • Registered dietitian
  • Home care nurse or coordinator
  • Driver
  • Personal care assistant
  • Center manager

Schedule a consultation. We’ll connect you with a member of our enrollment team over the phone to explain the program and answer any questions. Then we’ll follow up with an in-person home visit and assist you every step of the way through the application process.

As a nursing home alternative, LIFE Connection offers care in your home and at the LIFE Connection Center, as well as at contracted facilities such as hospitals, specialist offices, rehabilitation centers and — when needed — nursing facilities. Learn more about our LIFE Connection Center here.

As a PACE program, LIFE Connection is covered by Medicare and Medicaid. There are no out-of-pocket charges if you qualify for both Medicare and Medicaid, or for Medicaid only. For participants who do not qualify for Medicaid, there is a flat monthly fee. Our enrollment team can help you apply for Medicaid.
