Am I eligible for AtlantiCare LIFE Connection?

You are eligible for our elder care services if you:

• Are age 55 or older
• Live in Atlantic or Cape May County
• Meet eligibility criteria for nursing facility level of care
• Have been assessed by our care team and deemed able to live safely in the community with our helpful services

• I want a direct number to reach a person when I have questions regarding my care.
• I want to be able to reach my doctor on nights, weekends and holidays.
